These forms are for the 2021-2022 chartered clubs that are looking to be rechartered. Please do NOT fill out these forms unless your club was chartered in the 2021-2022 school year. There will be an informational meeting at the beginning of the 2022-2023 school year for new clubs that want to be chartered.
